Hormonal Mechanisms of Diabetes Control after Bariatric Surgery

RESUMO
A cirurgia bariátrica é um método eficiente para o tratamento da obesidade, com resultados benéficos tanto na perda do excesso de peso quanto no controle glicêmico. As Hipóteses do Intestino Proximal e Distal foram propostas para explicar os rápidos efeitos da cirurgia na melhora do controle da diabetes mellitus tipo 2 (DM2). Segundo essas hipóteses, a melhora no controle glicêmico está relacionada à exclusão do intestino proximal e à antecipação do distal, reduzindo a secreção dos fatores anti-incretínicos e aumentando os níveis dos hormônios intestinais. O peptídeo semelhante ao glucagon 1 (GLP-1) aparece como um dos hormônios responsáveis por essa melhora, devido a sua ação benéfica nas células β e secreção e ação da insulina. Entretanto, mais estudos são necessários com objetivo de avaliar os efeitos da cirurgia bariátrica no tratamento DM2, a fim de elucidar os mecanismos hormonais ligados à melhora do metabolismo da glicose.
ABSTRACT
The bariatric surgery is an effective method for treating obesity, with beneficial results in weight loss and glycemic control. Foregut and Hindgut Hypothesis were proposed to explain the rapid effects of bariatric surgery in the improvement of the Type 2 diabetes mellitus control (T2DM). According to these hypotheses, the improvement in glycemic control is related to the exclusion of the proximal intestine and anticipation of the distal, reducing the secretion of anti-incretin factors and increasing levels of gut hormones. The glucagon-like peptide-1 (GLP-1) appears as the hormone responsible for this improvement because of its beneficial action in β cells, and in secretion and insulin action. Further studies are needed to assess the effects of bariatric surgery in the cure of T2DM in order to elucidate the hormonal mechanisms linked to improved glucose metabolism.
