The use of Enteral Nutrition Therapy with Immune Nutrients in Critical ill Patients.

O uso enteral de alguns nutrientes específicos vem sendo recomendado para melhorar o estado geral do paciente crítico. O objetivo deste trabalho foi estudar os efeitos da glutamina, arginina e ácido graxo ômega 3 via enteral nestes pacientes, englobando 46 estudos publicados entre 1987 a 2009. A administração de glutamina interfere na digestão e absorção de nutrientes. Já a arginina, possui funções como cicatrização de feridas, ganho de peso pós-trauma e melhora da retenção nitrogenada. Ambos apresentaram estudos que demonstraram haver redução de complicações infecciosas e tempo de hospitalização quando administrados, porém apenas a arginina apresentou aumento na mortalidade. Os ácidos graxos ômega 3 possuem propriedades antiinflamatórias, antitrombóticas, antiarrítmicas e hipolipidêmicas. Apesar da escassez dos estudos que os utilizaram isoladamente, efeitos benéficos foram observados. Mais estudos são necessários para demonstrar os reais benefícios da imunonutrição nestes pacientes, pois as doses recomendadas e o tempo de utilização não estão claramente definidos.

The use of some specific nutrients in enteral nutrition therapy has been recommended to improve the general condition of critically ill patients. The objective of this presentation is to study the effects of glutamine, arginine and omega 3 fatty acid in these patients in enteral nutrition therapy, comprising 46 studies published between 1987 to 2009. The glutamine administration interferes in nutrients digestion and absorption. Arginine has functions such as wound healing, post-trauma weight gain and improved nitrogen retention. In both, studies demonstrated reduction of infectious complications and length of stay in hospital when administrated, but only arginine had increased mortality. The omega 3 fatty acids have anti-inflammatory, antithrombotic, antiarrhythmic and hypolipidemic properties. There are few studies that used it alone, but beneficial effects were observed. More studies are necessary to demonstrate the immunenutrition benefits in these patients, because the recommended doses and the time of administration are not clearly defined.
