Performance of the Nutrition in Hemodialysis Patients with Hyperphosphatemia

Introdução: O controle do fósforo sérico é um desafio no tratamento de pacientes em hemodiálise. O emprego de estratégias educativas poderia contribuir para melhorar a adesão destes pacientes ao tratamento. Assim, o objetivo deste estudo foi avaliar o impacto da atuação de um nutricionista em um centro de tratamento de doenças renais. Métodos: foram incluídos 31 pacientes [19homens/12mulheres, idade= 51,5±31,5 anos] que estavam no programa de hemodiálise durante o período de janeiro a dezembro de 2007 e com pelo menos 6 meses de programa. Foram medidas as concentrações séricas de fósforo, cálcio e a relação cálcio e fósforo. Resultados: após a orientação realizada por um nutricionista aos pacientes houve uma melhora significativa nos exames laboratoriais mensais, o que favoreceu para a diminuição dos fatores de risco. Assim, ocorreu redução do fósforo sérico (5,32±3,48 mg/dl para 4,62±2,6mg/dl; P<0,01), aumento do cálcio sérico (8,88±1,8 mg/dl e 9,1±1,5 mg/dl) e consequente melhora dos níveis do produto cálcio e fósforo (47,24±46,74 mg/dl e 42,04±34,49 mg/dl; P<0,01). Conclusão: a aplicação de uma educação nutricional resultou em melhora dos valores de exames bioquímicos mensais, os quais mostram a aderência e compreensão do paciente ao tratamento prescrito.

Introduction: The control of serum phosphorus is a challenge in the treatment of hemodialysis patients. The use of educative strategies could contribute to improve the adhesion of these patients to the treatment. Therefore, the objective of this study was to evaluate the impact of the performance of a nutritionist in a Kidney disease treatment center. Methods: 31 patients [19men/12women, 51,5±31,5 years] who were in the program of hemodialysis had been included during the period from January to December of 2007 and with at least 6 months of program. The serum concentrations of phosphorus, calcium had been measured and the relationship calcium X phosphorus. Results: After the orientation given by a nutritionist to patients there was a significant improvement in the monthly examinations, what helped to the reduction of the risk factors. Therefore, there was the reduction of the serum phosphorus (5,32±3,48 mg/dl to 4,62±2,6mg/dl; P<0,01), the increase of serum calcium (8,88±1,8 mg/dl and 9,1±1,5 mg/dl) and consequent improvement of the levels of the calcium phosphorus product (47,24±46,74 mg/dl and 42,04±34,49 mg/dl; P<0,01). Conclusion: The application of a nutritional education resulted in the improvement of the values of monthly biochemists examinations, which show to the understanding and agreement of the patient to the prescribed treatment.
