Vitamin B1 Deficiency Neuropathy in the Post-Operatory of Bariatric Surgery: A Case Report With Atypical Symptoms.

A deficiência de nutrientes após a cirurgia bariátrica pode resultar em diversas manifestações clínicas. A neuropatia periférica trata-se de evento menos comum, mas de graves consequências, e geralmente é ocasionada pela deficiência de tiamina. Este trabalho tem como objetivo apresentar o caso clínico de paciente do sexo feminino, 39 anos, submetida ao bypass gástrico em Y-de-Roux em 2007. Após três anos de cirurgia, esta iniciou com sintomas de dores musculares e parestesia de membros. Foi iniciado o tratamento pelo serviço de nutrição, com reposição de nutrientes e reestruturação da dieta. Com a continuidade do tratamento, houve uma melhora quase total dos sintomas. Pode-se afirmar com este estudo que a suplementação precoce e adequada é o que fará a diferença na evolução do quadro clínico, evitando complicações mais sérias, tais como perdas neuro-motoras definitivas.

The micronutrient deficiencies post bariatric surgery may result in many clinical manifestations. The peripheral neuropathy is a lesser common event, but with severe consequences, and it is usually caused by thiamine deficiency. The purpose of this article is to present the clinical case of a female patient with 39 years old, who underwent a Y-en-Roux gastric bypass in 2007. Three years after surgery, she presented symptoms of muscle soreness and limbs paresthesia. The treatment was initiated by the nutrition service with nutrient reposition and dietetic restructuring. With the sequence of the treatment, there was a significant improvement from all the symptoms. It can be stated by this study, that early and adequate nutrient supplementation is the key point in the clinical status evolution avoiding further serious complications, such as definitive neuromotor impairment.
